Privacy Policy
Linden Family Health is a HIPAA-covered healthcare provider. All protected health information (PHI) we collect — including your medical history, lab results, prescriptions, billing records, and portal messages — is handled under the standards set by the Health Insurance Portability and Accountability Act (HIPAA) and California's Confidentiality of Medical Information Act (CMIA). We use, disclose, and store PHI only for treatment, payment, and routine healthcare operations, except where you've given us explicit written authorization or where disclosure is required by law.
Beyond PHI, we collect basic operational data: contact details you provide when booking, the device and browser you use to access our patient portal, and aggregated analytics about how the portal is used (so we can improve it). Portal data is encrypted in transit and at rest, hosted with a HIPAA-compliant U.S. provider, and access-logged at the row level. We don't sell patient data. Ever. We don't use it for advertising. Ever.
We share data with third parties only in three cases: (1) business associates who help us deliver care — labs, e-prescribing networks, our portal vendor — all under signed Business Associate Agreements; (2) your insurer, for billing and authorization, with the minimum necessary information; and (3) law enforcement or public health agencies when legally compelled. You can request a copy of your records, request a correction, or request an accounting of disclosures at any time — email privacy@lindenfamilyhealth.example.
Accessibility Statement
Linden Family Health is committed to making our clinic, our patient portal, and our website usable by everyone, including people with disabilities. Our digital properties are built to conform with the Web Content Accessibility Guidelines (WCAG) 2.1 Level AA. We test with screen readers (VoiceOver, NVDA), verify color contrast, build for keyboard navigation, and provide text alternatives for images and media. Our clinic is wheelchair-accessible, with accessible exam rooms, an accessible restroom, and on-site parking with two ADA-designated spaces.

Terms of Service
These terms govern your use of lindenfamilyhealth.example and the Linden Family Health patient portal. By using either, you agree to these terms. They cover what we offer through the site (information about our clinic, scheduling, secure messaging for established patients), and what we don't (the website does not provide medical advice, diagnosis, or treatment, and using it doesn't establish a doctor-patient relationship — that begins only when you become an established patient through an in-person visit).
You agree to use the site and portal lawfully and not to attempt to access data that isn't yours, disrupt the service, or scrape it. We may suspend or terminate portal access if you violate these terms, breach the security of another patient's data, or use the service in a way that risks harm. We may update these terms occasionally — when we do, we'll post the change date at the top and, for material changes, notify portal users by message.
These terms are governed by the laws of the State of California, without regard to conflict-of-laws principles. Any dispute will be resolved in the state or federal courts located in San Francisco County, California. If any part of these terms is found unenforceable, the rest remains in effect.
